The pharmaceutical regulatory landscape has undergone substantial transformation over the past three decades, with regulatory authorities increasingly adopting “lean” operational models in response to resource constraints, scientific complexity, and demands for expedited market access. This perspective article examines the governance frameworks underpinning major regulatory bodies, particularly the US Food and Drug Administration (FDA) and the UK Medicines and Healthcare products Regulatory Agency (MHRA), and analyzes how lean regulatory approaches have fundamentally impacted the regulator-industry relationship. We explore the paradox of maintaining regulatory independence while fostering collaborative paradigms, the implications of resource-constrained oversight on public health protection, and the emerging challenges posed by adaptive licensing, accelerated approval pathways, and industry-funded regulatory activities. Through critical analysis of current governance models, we identify key tensions between efficiency imperatives and traditional regulatory safeguards, proposing a framework for sustainable regulatory excellence that balances innovation facilitation with robust public health protection. This perspective contributes to ongoing discourse on regulatory science governance and offers insights for policy development in an era of unprecedented biomedical innovation.
